1. An insurance contract entered in the separate sheet with regard to the claim for insurance proceeds for the death of public transportation accidents by the Defendant-Counterclaim Plaintiff.

The principal lawsuit and counterclaim shall also be deemed to have been filed.

1. Basic facts

A. On June 16, 2015, the Plaintiff entered into an insurance contract with the deceased as the insured on the attached list indicating that the representative beneficiary is the Defendant upon the death (hereinafter “instant insurance contract”). The main contents are as follows.

The term "public transportation disaster" in this contract refers to a disaster referred to in the Public Transportation Disaster Classification Table (See attached Table 3), and the term "traffic disaster" refers to a disaster referred to in the attached Table 3 of the Public Transportation Disaster Classification Table (see attached Table 4), and the term "traffic disaster" refers to a disaster referred to in the attached Table 4 of the Traffic Disaster Classification Table (see attached Table 4).

Article 5 (Grounds for Payment of Insurance Money) If any of the following events occurs to the insured, the Company shall pay the insured insurance money agreed in the Schedule of Standards for Payment of Insurance Money (See attached Table 1):

3. Where the insured dies from a public transportation disaster that occurred during the insurance period: Payment of the insurance proceeds for death of a public transportation disaster;

4. Where the insured dies due to a traffic accident other than a public traffic accident that occurred during the insurance period: Article 10 (Procedures for Payment of Insurance Money) (1) The company shall, upon receipt of the documents prescribed in Article 9 (Request for Insurance Money), send the receipt thereof, cellular phone text messages, emails, etc., and shall pay the insurance money or exempt the payment of the insurance premium within three business days from the date of receipt of such documents.

Provided, That if it is necessary to investigate or confirm the grounds for the payment of insurance proceeds and the grounds for exemption from the payment of insurance premiums, the insurance proceeds shall be paid or the insurance premiums shall be exempted.

3. Public transportation disaster death insurance proceeds (Article 5 subparagraph 3): The insured worker shall have occurred during the insurance period.
