1. Counterfeiting a private document written application for subscription to U mobile phone and using a falsified document;

A. On April 23, 2016, the Defendant forged a private document: (a) entered “U”, “V”, “V”, “W” in the customer address column, “W” in the contact address column; and “U” in the applicant column; and (b) signed “U” in the applicant column on the applicant column, by means of a verification color pen on the written application form for mobile phone joining the mobile phone.

As a result, the defendant has forged the application form for mobile phone membership in U.S. for the purpose of exercising rights and duties.

B. The Defendant, at the time and place specified in 1.A., sent the forged U.S. application to an employee under the name of the LGU who was unaware of the forgery through facsimile as if the document was duly formed.

2. Counterfeiting a private document of an application for purchase of a mobile phone in X-X and using a falsified document;

A. On September 21, 2016, the Defendant: (a) disclosed X’s personal information to the H office located in the Taedong-gu in the World War (Seoul); and (b) instruct the Z working in the same office to prepare a criminal record of carrying in the name of the female; and (c) again, the Z used the verification body in the form of the application for mobile phone subscription to the mobile phone to write “A” in the name of the subscriber in the name of the subscriber, “B, 201”; (d) written “AC” in the column of the Seo-gu Office of Customer Address; and (e) written “AC” in the column of the date of preparation, “X” in the column of the applicant’s name.
