1. On June 30, 2016, the Defendant forged an application form for cell phone subscription, which is a private document on the rights and obligations, with the intention of exercising the signature on the applicant’s name and affixed it to the “E”, “F”, “B”, and “E” in the column of customer information on the site of “E” at the mobile phone sales store located in the large exhibition medium-gu B operated by the Defendant, and submitted it to the D Co., Ltd. as if the document was duly formed.

2. On July 29, 2016, the Defendant entered “G”, “E”, “F” in the subscriber name column, and “E” in the applicant column in the “application form for change of D compensation period” at the same place as above, and submitted one copy of the application form for change of compensation period, which is a private document pertaining to rights and obligations, to D corporation, as if it were a document duly formed, for the purpose of exercising the signature by signing the application form next to the name.

3. On October 29, 2016, the Defendant: (a) stated “G”, “E”, “E”, “F”, and “E” in the column for the mobile phone call on the mobile phone page of “D” at the aforementioned location; (b) forged one copy of the application form for the mobile phone purchase, which is a private document related to rights and obligations, for the purpose of exercising rights and obligations by signing it on the name side; and (c) submitted it to D Co., Ltd. as if the document was duly formed.

A. On June 29, 2016, the Defendant entered “C” Handphone sales stores located in Daejeon Daejeon-gu, Daejeon-gu, in an application for change of H mobile service, without authority, and entered “E” and “F” in the applicant column, and entered “E” in the applicant column.
