A defendant shall be punished by imprisonment for one year.
However, the execution of the above punishment shall be suspended for a period of two years from the date this judgment becomes final and conclusive.

1. Forging a private document;
A. On August 24, 2015, the Defendant: (a) drafted two copies of “D” application form using a verification color pen in Seo-gu, Seo-gu, Daejeon; (b) entered “D”; (c) the subscriber’s address column; and (d) written “D” in the customer’s customer column; and (c) written one copy of “D” in the applicant column; (d) signed the “D” on each side of the name; (d) write the applicant’s signature in the applicant column; and (d) written one copy of “D’s consent to use personal information; and (e) written one copy of “D” written in the applicant column, stating “D” in the applicant column, stating “D” signature in the applicant column; and (e) written the applicant’s signature in the name “D” column, stating “D’s signature; and (e) written the name “D” column in the “D” column, stating the applicant’s signature in the name “B” column.
Accordingly, for the purpose of uttering, the Defendant respectively provided two new service contracts in the name of D, which are private documents related to rights and obligations, one copy of personal information utilization consent form, one copy of LTE Plus/3 application form for LG Phone Customer Agreement form, one copy of LG Phone Customer Agreement form, and one copy of application form for selective contract discount program form.
B. On December 10, 2015, the Defendant related to the forgery of the I’s name prepared two copies of “A mobile phone membership application” using a verification color pen in the Seo-gu Daejeon District Court on the following: (a) written in the buyer column “I”; (b) written in the subscriber column “L”; (c) written in the customer address column; (d) written in the customer address column; and (e) written in the customer column, the signature of each “I”; and (e) written in the applicant column “I”; and (e) written in the copy of “A consent to use personal information.”
