1. With respect to each insurance contract listed in the separate sheet 1, the Plaintiff due to an insured event listed in the separate sheet 2.

1. Basic facts

A. On April 16, 2014, the Plaintiff entered into an insurance contract with the Defendant as the insured each of the insurance contracts listed in the attached Table 1 (hereinafter “instant insurance contract”). The main contents of the cancer diagnosis cost and cancer surgery clause (Renewal) contained in the instant insurance contract are as follows.

Article 2 (Detailed Provisions concerning Payment of Insurance Money) (1) Even if the diagnosis has become final and conclusive due to the Class A cancer, other skin cancer, job cancer, or boundary pattern, the insurance money for general cancer shall be paid in accordance with the terms and conditions of this Special.

However, after the general cancer has been diagnosed and confirmed, it is not possible to pay insurance proceeds for the Athroid cancer, other skin cancer, job cancer, or boundaryal pattern, even if the diagnosis and determination is made with the Athroid cancer, and then for the Athroid cancer, job cancer, or boundaryal pattern.

Article 3 (Definition and Determination of Maternity Cancer, General Cancer, Maternal Cancer, Job Cancer, and Boundary Species) (1) The term "Maternity" in this Special Terms and Conditions shall be the disease (see attached Table 6 (Maternity Classification Table 6) which is divided into malicious life (Maternity 6) in the 6th Korean Standard Disease Classification and Death Classification.
