1. Basic facts

A. On September 8, 2008, the Plaintiff concluded an insurance contract with the Defendant as the insured and entered into an insurance contract with the same content as the attached Form (hereinafter “instant insurance contract”).

B. The Defendant was hospitalized for 589 days in total due to cleaning or traffic accidents as indicated below, and was paid insurance proceeds of KRW 40,857,998 by claiming 15 insurance proceeds from the Plaintiff according to the instant insurance contract.
