1. It is true that an insurance contract entered in the separate sheet concluded on April 30, 2014 between the Plaintiff and the Defendant exists.

1. Basic facts

A. (1) On April 30, 2014, the Plaintiff entered into an insurance contract with the Defendant (hereinafter “instant insurance contract”) with the same content as the attached list, and paid the first insurance premium. (2) The main part of the terms and conditions of the instant insurance contract (hereinafter “instant insurance contract”) relating to the issues of the instant case is as follows.

Article 15 (Obligation to Notify before Contract) The contractor or the insured shall be informed of the fact that he/she knows about the matters asked in the written subscription (in the case of a diagnosis contract, the time of health diagnosis) when he/she makes an offer (in the case of a diagnosis contract, the time of health diagnosis).

Article 17 (Effect of Violation of Obligation to Notify) (1) The Company may terminate this contract regardless of the occurrence of damages, in the following cases:

At the time when the company becomes aware of the fact, or when two years have passed without any cause for the payment of insurance proceeds from the time when the company received the first insurance premium, the Plaintiff responded to the following questions: (i) The Plaintiff, within the last three months, at the time of the conclusion of the instant insurance contract, provided that the Plaintiff is obligated to notify the contract before the conclusion of the contract, which is attached to the written subscription.
