1. Basic facts

A. On November 6, 2009, the Plaintiff and Defendant A entered into the instant insurance contract with Defendant A as the insured and beneficiary (hereinafter “instant insurance contract”), and on January 21, 2010, the policyholder and beneficiary of the instant insurance contract changed to Defendant B (Defendant B’s ASEAN).

B. Defendant A’s hospitalized treatment for 27 days from October 8, 2010 to November 3, 2010, which was after the instant insurance contract, was received from C Hospital to “malinal viral viral life of Apryp,” as shown in attached Table 2, as well as from around that time to September 3, 2015, Defendant A was hospitalized for 1,182 more than 46 times in total.

Among the insurance contracts that Defendant A as the insured, and the insurance contracts whose daily days of hospitalization are guaranteed by the Defendant A as the insured and whose payment of insurance money is guaranteed by disease, the insurance contracts maintained until the time of the instant insurance contracts, and the insurance money that the Defendants received pursuant thereto are as follows.

The entry of an insurance company’s “stock company” below the sequence is entirely omitted.

Where it is possible to know the standard amount of monthly insurance premium (original insurance premium) that is the date of the contract, it shall be followed, and where it is impossible to know, the result of the order to submit financial transaction information shall be followed.

Insurance money means all insurance money paid under the relevant insurance contract, including daily allowances for hospitalization.
