1. It is confirmed that an insurance contract entered in the separate sheet concluded between the Plaintiff and the Defendant is null and void.
2. The defendant.
Reasons
1. Basic facts
A. On November 26, 2008, the Defendant entered into an insurance contract (hereinafter “instant insurance contract”) with the Plaintiff as indicated in the separate sheet with which the insured is himself/herself (hereinafter “instant insurance contract”).
B. From December 20, 2008 to January 10, 2009, after the conclusion of the instant insurance contract, the Defendant received hospital treatment for 22 days in total 823 days from December 20, 2008 to December 25, 2014 as shown in Table 1 as follows.
