The defendant's refusal to request the disclosure of information on the information listed in attached Form 1.1, which the plaintiff rendered on August 29, 2012, shall be revoked.
2. Purport of appeal
The part against the defendant in the judgment of the first instance shall be revoked, and the part of the plaintiff's claim shall be dismissed.
