1. Basic facts

A. On December 14, 2010, the Defendant entered into an insurance contract in the same E as the attached Form (hereinafter “instant insurance contract”) with the Defendant, the insured, the insurance period from December 14, 2010 to December 14, 2068 (hereinafter “the instant insurance contract”). According to the said contract, when the Defendant was diagnosed and confirmed to be brain stroke during the insurance period of the special terms and conditions for guaranteeing brain strokes, the Defendant would pay the brain strokes amounting to 100% of the purchase amount of the insurance (30 million won) only once for the first time.

(2) The diagnosis and confirmation of brain shall be conducted by a person with a certificate of qualification of a domestic hospital prescribed in Article 3 (Medical Institutions) of the Medical Service Act or a foreign medical institution abroad recognized by the company as equivalent thereto, and such diagnosis shall be conducted based on medical records, computerized single-rise photography (excluding dentists), nuclear celebrology image method (MRI), cerebral celebrology and electronic emission celebrology (PET), celebrology and electronic emission celebrology (PEC), celebrology and celebrology test, etc.
