1. Basic facts

A. On April 30, 2009, Defendant B concluded an insurance contract on April 30, 2009 with Defendant C as the insured (hereinafter “instant insurance contract”).

B. On October 5, 2009, after the conclusion of the instant insurance contract, Defendant C received hospitalized treatment for two days as an cromatic stone on October 5, 2009, as indicated in the attached Table 2, including the details of hospitalized treatment, five times in total from October 5, 2009 to April 5, 2016 (hereinafter “instant hospitalized treatment”).

According to the instant insurance contract, the Plaintiff paid the Defendant C totaling KRW 12,438,098 as indicated in the insurance proceeds for payment of the attached Form 2.
