1. Basic facts

A. Nonparty B (hereinafter “the deceased”) is an insured or policyholder who has entered into a high trust and life insurance contract (attached Form No. 1; hereinafter “instant insurance contract”) with the Plaintiff. The Defendant is a beneficiary designated to receive death insurance money at the time of the death of the insured pursuant to the instant insurance contract.
