1. 사문서변조 피고인은 2018. 9. 18. 17:00경 인천 부평구 B 소재 피고인이 은행원으로 근무하는 C은행 부평지점에서, 피해자 D 명의 ‘신용카드 가입신청서’의 카드구분란에 국내전용 항목에 ‘∨’ 표시되어 있는 것을 ‘X’ 표시로 수정하고 국내외겸용 항목에 ‘∨’ 표시한 후 ‘VISA’라고 기재하였고, 개인(신용)정보 선택적 수집ㆍ이용에 관한 사항란에 4가지 선택항 중 마지막 항인 ‘본인은 귀사가 상기 목적으로 다음과 같은 본인의 고유식별정보를 처리하는 것에 동의합니다’ 부분에 ‘동의하지 않음’ 항목에 ‘∨’ 표시되어 있는 것을 삭제의 의미로 두 줄을 긋고 위 4가지 선택항 모두 ‘동의’ 항목에 ‘∨’ 표시하고, 개인(신용)정보 선택적 제공에 관한 사항란에 3가지 선택항 중 마지막 항인 ‘본인은 귀사가 상기 목적으로 다음과 같은 본인의 고유식별정보를 처리하는 것에 동의합니다’ 부분에 ‘동의하지 않음’ 항목에 ‘∨’ 표시되어 있는 것을 삭제의 의미로 두 줄을 긋고 위 3가지 선택항 모두 ‘동의’ 항목에 ‘∨’ 표시를 하는 등 행사할 목적으로 피해자 명의의 신용카드 가입 신청서를 변조하였다.

2. The Defendant used the altered private document by means of cans and transmitting the altered private document to C card as if the application form for subscription under the victim’s name was prepared normally at the aforementioned temporary location.
