1. On the basis of an insurance contract listed in the attached Form 1, it shall be due to an accident described in the attached Form 2;

A principal lawsuit and counterclaim shall also be deemed a principal lawsuit and counterclaim.

1. Basic facts

A. On April 18, 2013, the Plaintiff entered into the instant insurance contract (hereinafter “instant insurance contract”) with the Defendant, as shown in the separate sheet No. 1 attached hereto.

The main contents of the insurance contract are as follows:

When a state of disability equivalent to a rate of disability payment which is less than 80% specified in the disability classification table during the insurance period, the insurance proceeds for general aftermath disability (the amount calculated by multiplying the payment rate prescribed in the disability classification table by the insurance amount entered in the insurance policy) shall be paid.

Among the insurance terms and conditions of the insurance contract of this case, the disability classification table for arms and fingers shall be as follows, and the rate of payment of disability for the first parts and fingers shall, in principle, be added up respectively, but the rate of payment shall not exceed 60%.

(see section 4-8-11 of A). When the defendant receives required documents when claiming insurance proceeds, a certificate of receipt shall be issued, and the insurance proceeds shall be paid within three business days from the date of receipt of the documents.
