1. With respect to the accident described in attached Form 2, the Plaintiff’s Defendants based on the insurance contract as stated in attached Form 1.

1. Facts of recognition;

A. On January 2006, the Plaintiff entered into an insurance contract with the Army (a public official organization insurance contract; hereinafter “instant insurance contract”).

The terms and conditions of the above insurance contract relating to this case are as follows:

[General Terms and Conditions of Government Officials Group Insurance] Article 1 (Establishment of Contract) (5) This Public Official Group Insurance Contract shall apply to any of the following contracts:

1. The insured shall be a member of a public official organization and the insured shall be applicable in cases where there are five or more persons;

2. A person who has become a contractor on behalf of the representative of the organization under subparagraph 1 shall be able to exercise all the rights and obligations under the group insurance contract.

3. Public officials' organizations referred to in subparagraph 1 mean central administrative agencies, special local administrative agencies, subsidiary agencies, advisory agencies, subsidiary agencies, assistant agencies and subordinate organizations which are established under the Government Organization Act and other Acts and subordinate statutes and prescribed by the General Rules on the organization and quota of administrative agencies.

[Special Terms and Conditions of Insurance for Public Officials - Special Terms and Conditions of Insurance for a Disease Death or a Disability with at least 80% of a disease, etc.] Article 1 (Types and Grounds for Payment of Insurance Money) Company is entitled to insurance money under this Special Terms and Conditions to the beneficiary (person who receives insurance money) when any of the following events occurs to the insured (person subject to insurance).

1. If the person dies of a disease during the insurance period: Death insurance proceeds;

2. When the rate of disability payment specified in the Disability Classification Table becomes at least 80% due to a disease diagnosed and confirmed during the period of insurance: the amount of disability insurance (the scope of this special terms and conditions) shall be applied at the time of concluding the general terms and conditions and other special terms and conditions, and shall be subject to the following conditions:
