A. During the period from November 15, 2013 to December 18, 2014, the Plaintiff undergone the sex surgery for the following beauty art purposes at the Defendant’s “Csung surgery (hereinafter “Defendant Hospital”)” operated by the Defendant, and the process of the surgery is as follows.

1) 코 수술, 이마 보형물 삽입술 가) 2013. 11. 15. 1차 코 수술 및 이마 보형물 삽입술 - 코 끝에 귀 연골을 삽입, 이마에 실리콘 보형물 삽입 - 수술 후 10일째 우측 콧속에 출혈이 지속적으로 있어 바세린 거즈 넣음 - 수술 후 1개월째 우측 콧날개 상부와 좌측 콧등에 TRA 나) 2014. 3. ~ 2014. 4.경 2차 코 수술 - 콧대 흔들리지 않게 고정, 두툼한 부위 줄이는 수술 다) 2014. 6. 3. 3차 코 수술 - 코 보형물 위치 고정 및 양측 절제 - 3차 수술 후 10일째 코끝이 빨갛고 부음. - 3차 수술 후 1달째 우측 콧속 위쪽 붉고 통증 있어 항생제 처방 - 3차 수술 후 2달 22일째 우측 콧날개 상부 비후 라) 2014. 12. 18. 4차 코 수술 - 코끝 줄임, 귀족수술(콧망울 주변에 보형물을 넣어 패인 부분을 융기시키는 수술) - 4차 수술 후 4일째 좌측 콧속 상처 거즈 넣음. - 4차 수술 후 1달 22일째 좌측 귀족수술 절개부위 피딱지 앉고 진물 흐르는 느낌 있어 내원, 진료 후 큰 이상 없어 특별한 처치 하지 않음 - 4차 수술 후 8개월 7일 째 좌측 귀족수술 삽입물 움직이고 우측 콧날개 상부 비후 지속되어 내원 2) 눈 수술 가) 2014. 12. 28. 1차 눈수술 - 좌측 상안검외안각가측 흉터 발생 나) 2015. 8. 25. 2차 눈수술 - 양측 눈끝 라인 교정술 다 2016. 4. 13. 3차 눈수술 - 좌측 상안검외안가측 흉터와 쌍커풀 라인을 연결하는 수술
