1. Claim by a participant in the plaintiff's succession

A. Indication of Claim - Attached Form 1 (Provided, That “creditor” shall be “Plaintiff” and “debtor” shall be deemed “Defendant”) and the reasons for participation shall be as indicated in the attached Form 1.
